There is a $175.00 filing fee for a Petition to Appoint Conservator of a Minor.
Certified Letters of Authority are an additional $11.00 each.

Nominated Conservator Agreement
There are two forms the must be completed and filed with the court within 28 days of qualification:
- Inventory: Completed by the conservator.
- Proof of Restricted Account: Completed by financial institution holding the minor’s funds. (This form cannot be filled out by the conservator.)
An Annual Verification of Funds on Deposit must be filed annually. Your due date is the date you are appointed.
If your household income us under 125% of the federal poverty guidelines, you may file a Fee Waiver Request.
