Annual Report: An Annual Report of Guardian on Condition of Legally Incapacitated Individual is due annually on the date of appointment.
- If you are suspended: You must file a Petition and Order for Reinstatement along with the report(s) that are delinquent. There is a $20.00 filing fee for the Petition and Order for Reinstatement.
Accounts: An Account of Fiduciary is due annually on the date of appointment, if accounts are ordered by the judge.

If you are suspended: You must file a Petition and Order for Reinstatement along with the account(s) that are delinquent. There is a $20.00 filing fee for the Petition and Order for Reinstatement and a $20.00 filing fee for each account filed.
If your household income us under 125% of the federal poverty guidelines, you may file a Fee Waiver Request.
